Nominations for any of the 20th Anniversary Recognition Awards can be made on-line using the form below or submitted using the PDF version of the nomination form.

The deadline for submissions is 1 June 2007:

Persons may nominate themselves or their own organization, or they may choose to nominate a third party who they believe should receive recognition. All nominations must be supported by a summary of up to 300 words providing support, including concrete examples of unique contributions of the individual, entity or project.
